NEW PATIENT FORMS

While traditionally most medical offices have you fill out paper forms, we are 100% paperless. All of our paperwork is filled out via your mobile phone or via a tablet in our office. We request that you fill out your new patient forms before visiting one of our offices.

WHAT TO EXPECT

On your first visit to Smile Team, we will review your dental and medical history form, your dental x-rays (if you have ones sent to us from another office) or we will take new x-rays if required. Then, one of our Dentists will perform an exam and discuss all the findings and present all options for treatment (if treatment is needed). After that your Dentist will answer all your questions or concerns. Finally, your Dentist will perform a dental cleaning if no gum disease is present.  To be as effective as possible, you may want to bring a list with your dental health questions to your visit to make sure they are addressed by your Dentist.

In order to avoid unnecessary x-rays, please obtain any current x-rays from your previous dental provider before your appointment with Smile Team. X-rays should be emailed to team@gosmileteam.com. If you have any questions, please call our office at 215-709-0001.

We understand that your availability can change at the drop of the hat, but we kindly request that you provide the office 24 hours notice if you are unable to make your appointment. Please give us a call to reschedule.
